Introducing the Haven Uro-Oncology Program with Alan Birch
This video is a recording of Prostate Cancer Foundation Canada's June 2024 Advanced Prostate Cancer Support Group meeting held over Zoom.
Alan Birch is the Associate Director of Provider Solutions Oncology at Sentrex Health. He is also the Founder of Drug Access Canada. You can see more information on their website, DrugAccess.ca. It provides resources for patients and health care providers. Alan started as an Oncology Drug Access Navigator at North York General Hospital in Toronto where he worked in the cancer centre for 8 years. At Sentrex, Alan created the prostate cancer patient support program called Haven.

    I think Rons success at beating prostate cancer may be due to his running and less to the drugs. I have found the medical "experts" only focus on treatments and not cures. There is no money in curing cancer. Just my opinion. Often just discussing making changes to diet brings ridicule/how dare you question doctors and the discussion is SHUT DOWN on cancer forums.

    For any men with prostate cancer watching this, studies shown here are 15 years old. And if published in 2011 with 20 year survival data it means 1990 treatments were in effect… Prognosis is much better now than it was then.

    Hi Dr I'm from Durban south Africa I had 10 out of 14 cores test positive for Gleason 6 on biopsy - my psa has been ranging from 4.7 to 6.7 in the last three years also coming down at times - is there any hope for active surveillance or not

    I’m going to Duke, they never put on Vitamin D when I was on Lupron for one year now I have severe back pain. Cancer came back and they tried Lupron again. I can’t take it and I told them so. Gave it to me anyway.

    I'm going to start proton therapy pretty soon.And i'm frightful that them radiating in the area is just gonna disturb the cancer and it will spread. That's what I am a scared of. Then it will get worse. Everybody's story I hear it never goes away. Surgery, But radiation, A d t. It's a never ending process. That once the medical field gets their hooks in you they never let you go!!!!!

    Thanks for this. I'm not sure how hormone therapy impacts these numbers. For example. I am 4+3 = 7 with a large tumor covering almost the entire gland. Some suspicion of disease in pelvic nodes but not definitive even after PSMA PET. I have had 3 of my 6 month course of hormone therapy (Lupron plus Darolutamide trial). My psa went from 17.0 to 0.06 ng/ml on test immediately before radiation started. The plan is for External beam radiation of about 9 weeks. Start with wide field to get pelvic nodes, prostate and seminal vesicles. Finish with more narrow field to get prostate only (as I understand it anyway). Brachytherapy is not in my plan. Not even presented to me. Its hard to imagine my psa going any lower after radiation???

      My husband is 65, had a PSA of 4.7 (within normal limits for his age), but because it went up 2 points within 6 months of his last PSA, he had a biopsy. His Gleason score is 4+3=7, MRI, PSMA shown cancer only in the prostate. A 4K test showed it was less aggressive then what the Gleason score showed. He had one 6 month injection of Lupron. Weeks later, LDR brachytherapy, since the cancer was tiny he only had the minimum amount of 38 seed placements. Weeks after that, he's going to get 5 weeks of beam radiation therapy. The way the doctor explained it the radiation damages the DNA of all cells. First you knock cancer down with hormone therapy that starves the cancer cells from testosterone. Cancer cells weakness is repairing it's DNA, so it will begin to die off. Which can occur over several months, or longer. The normal cells that that has damage DNA from the radiation treatment, can eventually repair itself. The immune system will clean up the damaged dead cell debris left behind. After that it is active surveillance.

      I don't know if it helps cancer but I do know that in Idaho falls, by private pay, a session is 250 dollars at the hospital. In a private HBOT center in the same city it's 150 per session but their tank does not take to as low a pressure, but still good for quite few issues.

    I just bailed on my 3rd ADT shot (6 mo shot) when I dug into the cardio risks after 12 mo's .... mid risk gleason 7 (4+3) 28 rad treatments .... my Oncologist never pointed out this risk, but pushed for 18 mo's (eliguard shot is 3K

    This was a valuable presentation for me. The discussion of mortality in relation to recurrence was especially helpful. My biopsy on 2023-12-20 revealed 3 + 4 = 7, with one core 90% involvement and another had 70%, most others much lower. Half of my cores contained cancer. I'm opting for robot assisted prostatectomy on 2024-04-09. I am 64, athletic, optimal weight and in good health. I do have issues with blood clots so I will be going with an ICV filter. This presentation helps me gain a little more info about what I may be in for long term, thanks a lot.

      Had my surgery on April 22, doing well. Hoping PSA in July is where it needs to be, I really don't want ADT or radiation. Gleason grade 4, Gleason score 8, so we'll see. Hope you are doing well

The pathology report post surgery came back with negative margins and no cancer in the lymph nodes or seminal vesicles. I do have perineural invasion, don't know much about that.     Nearly a year ago, my wife died from liver cancer. During her illness , I had lost a lot of weight and had developed lower back pain, but was otherwise quite a fit 78 year old . A routine AP blood test indicated a possible cancer, but an ultrasound scan was negative., but my GP ordered a PSA test which was 872 (is that a record?). Our National Heath Service is marvellous, and within days assigned an oncologist. There and then, he told me that be believed I had advanced prostate cancer which was likely to have metastasised, Having a suite of scans was necessary because those procedures would have to be undertaken. All those tests, and biopsy confirmed it was it was Gleason 9 and had metastasised at least to my lungs and bones. Other than backache, mild problems with urination and the weight loss, I felt OK. The oncologist offered chemo or ADT, but a clotting disorder, a long prolonged AQT and my desire for quality of life, we agreed that I would go onto daily ADT (Xtandi) and a three monthly injection of Prostrap. My PSA rapidly fell to about 2. I experienced nearly all the side effects plus others (unbearable itching being one of them. I put back on half the weight I had lost but distributes to different places, and I joke that the NHS should provide me with a bra. Yes, mood swings, depression, fatigue etc have hit me hard, but I think that they are declining. I dont regret this way of managing the disease. I have chronic atopic eczema which I believe has been aggravated by the meds, a tapered course of steroids can remarkably effective. Our NHS has been remarkable and really quick to act. Maybe our consultants are too busy to spend much time with you, or show you the scans etc. Basically I have experienced more quick actions than long chats in the consulting room. What do you think?

    I have prostate cancer that metastasize to my spine and before it did metastasized my Dr told me to do ADT did not mention one word about it so I filled my prescription and went home and decided to educate myself on this medication he gave me … After extensive research I told my wife that there’s no way in hell I’m going to take the medication…. So today I’m just doing radiation and my faith will get me through the rest….

      I Just Went Through This Same Situation My Cancer Was All Still In My Prostate ..I Had Gleason 8 PC In One Core 7,6 In Two More Cores .Other 9 Were No Cancer .PSA Was 4.5 .I Had 4 Weeks EBT .I Refused To Take The ADT Firmagon Shot And Had Permanent Brachy Seed Implants Last June .two Weeks Ago My Oncologist Told Me That My PSA Was O.6 And A Week Later My Cancer Was Undetectable .In Remission.

      It is meant to be from a caregiver perspective. She does seem to experience high caregiver stress. Every caregiver is different, but I believe that as a spouse you have “co-cancer”.

    Active surveillance is the way to go. You cant use what you dont have. Like shooting pool with a rope. Everyone wants to try and live longer but its better to die with your boots on.

    I have had just one injection of ADT. I have decided to not have the further scheduled 5 doses. I had a small though aggressive prostate cancer. Treated with radiation. My PSA kept rising after 12 months. The special PET scan indicates 4 nodes now have cancer spread. I have been prescribed hormone tre without any information given to me. The side effects appear to outlay the treatment. I have chosen keto diet, vit D and more active life with less stress. Do you think I am going down the wrong track?

      Hi @12superoo, we can't provide medical advice, but would definitely suggest that you discuss your treatment decision further with your doctor, or get a second opinion as to what's recommended in your case from another healthcare professional, if possible. While there can be many side effects from ADT, there are also effective strategies to manage them, including those you have suggested - exercise, eating a low carb diet, and Vitamin D - that you may want to consider trying in conjunction with your treatment before deciding to go completely off of ADT.
